Weight management is about how your body uses energy

Weight is not only the number on the scale. It reflects how your body uses energy, stores fuel and responds to change. That is why I do not treat it as a willpower problem. I look at how your system is functioning first.

At NP Empowered Wellness in Roseville, I build medical weight management around your health history, lifestyle and goals, with an emphasis on muscle preservation, nutrition, strength, sustainable habits and clinical follow-up. It is never medication alone.

GLP-1 medications are valuable tools, not moral shortcuts. Handing someone medication without guidance is the problem, not the medication itself. When a GLP-1 is appropriate, I prescribe compounded semaglutide or tirzepatide as one part of a personalized plan, and I may use the brand-name Zepbound pathway through LillyDirect when it fits.

Large weight loss can include muscle loss, so protein, strength training and measurement matter. Plateaus and maintenance are expected parts of chronic weight care, and I plan for them from the start. There is no guarantee of weight loss, and I do not promise timelines.

If you live outside Roseville, you can start by telehealth where appropriate. I complete your intake assessment virtually, and patients across California can begin from home. Patients from Rocklin, Granite Bay and beyond are welcome either way.

Who a GLP-1 program may help

  • Adults with a BMI of 30 or higher, or 27 or higher with an obesity-related health condition
  • People who have struggled to lose weight with lifestyle changes alone
  • People with stubborn weight, low energy or appetite that is hard to manage
  • Patients who want fat loss while keeping muscle
  • Patients who want labs and monitoring, not a prescription alone

Weight management pricing

  • Appetite regulation
  • Metabolic support

Brand-name Zepbound pathway

I may use LillyDirect when it is appropriate for you. Insurance coverage and medication availability are not guaranteed.

Maintenance support

A major part of how I work. Maintenance may require ongoing medication, strength training, nutrition, monitoring and habit support, and we plan it together from the beginning.

  • Muscle preservation
  • Habit support
  • Monitoring

Body-composition follow-up

InBody measures muscle, body fat and body water, so progress means more than the number on the scale. Grip-strength testing is a simple measure connected to strength, function and healthy aging.

  • Muscle
  • Body fat
  • Body water
  • Grip strength

Vitamin injections and supplements

Vitamin B12, Lipo-C and glutathione injections are available, along with at-home B12 and at-home Lipo-B. I confirm the details and pricing with you at your visit. I may also recommend professional-grade supplements through Fullscript and Thorne. Supplements do not replace medical evaluation and do not guarantee outcomes.

Prices are starting points. Your exact plan, with the price of each step and the total, is written out at your consult. Cherry and CareCredit payment plans are available.

Foundation layer, Plan A: Inside first

Weight management sits in the foundation layer of Plan A, Inside first, because metabolism and hormones set how everything above them responds. It pairs well with hormone optimization and a lab review, and with hair support during weight loss. Skin treatments can wait until the base is working.

How the program works

01

Assess and understand

We start with a conversation about your weight history, goals and medical context, plus labs when they are clinically appropriate, so I understand how your body is functioning.

02

Build your plan

Based on what we learn, I build a personalized plan that may include medication, nutrition, strength training and supplement recommendations.

03

Build and implement

We build a repeatable plan around nutrition, protein intake and strength training to support fat loss and muscle preservation.

04

Refine and maintain

I monitor your progress, adjust the plan and move you into a long-term strategy that supports maintaining your results.

Before and after weight management

Before your appointment

  • Complete any baseline labs I ask for before you start. I confirm clearance with you.
  • Review your Program Guide in full so you know your starting dose and injection technique.
  • Store medication refrigerated and protected from light.
  • Use only the syringes and needles provided or recommended by your provider.
  • Choose one injection day each week and keep to it.

After your appointment

  • Inject subcutaneously into the abdomen, thigh or upper arm and rotate sites each week. Never inject into a vein or muscle.
  • Prioritize protein with every meal, with a daily target of 80 to 120 g, and drink 64 to 80 oz of water daily.
  • Move consistently. Strength training helps preserve lean mass. Avoid alcohol.
  • Nausea: eat small, bland meals and avoid fatty or spicy foods. Ginger tea, peppermint and B6 can help.
  • Constipation: increase water, fiber and movement. Diarrhea: avoid high-fat or high-sugar foods and stay hydrated.
  • Fatigue is common in the first 1 to 2 weeks. Rest, hydrate and keep your protein up.
  • With reduced appetite, do not skip meals entirely. Focus on nutrient-dense foods.

Weight management questions

What does it cost to get started?

A weight consult is $50 for 30 minutes, in Roseville or by telehealth when appropriate. Medication formulation, dose, availability and pricing can change, so I confirm medication pricing with you at your visit rather than quoting it here.

Does insurance cover GLP-1 weight loss medications?

NP Empowered Wellness is a cash practice and does not bill insurance, but HSA and FSA cards are accepted. If you choose the brand-name Zepbound pathway, I may use LillyDirect when it is appropriate. Insurance coverage and medication availability are not guaranteed.

Can I start a GLP-1 program by telehealth?

Yes, when it is appropriate for you. Telehealth is available across California. NP Sarah completes your intake assessment virtually, reviews your health history and, if appropriate, can prescribe through a telehealth visit. Wednesdays are telehealth only.

Is a GLP-1 medication a shortcut?

No. GLP-1 medications are valuable tools, not moral shortcuts. Handing someone medication without guidance is the problem, not the medication itself. That is why your plan includes nutrition, strength training, monitoring and maintenance.

What is the difference between semaglutide and tirzepatide?

Semaglutide is a GLP-1 receptor agonist that reduces appetite and slows gastric emptying. Tirzepatide is a dual GIP and GLP-1 agonist, so it works on two pathways for appetite and blood sugar regulation. NP Sarah helps you choose based on your goals, health history and what is appropriate for you.

What results can I expect?

Everyone responds differently. I do not guarantee weight loss or promise a timeline. Many people notice appetite changes first. Plateaus and maintenance are expected parts of chronic weight care, and your plan accounts for them. InBody measurements help us follow muscle and fat, not just the scale.

Will I lose muscle on a GLP-1 program?

Large weight loss can include muscle loss, so protein, strength training and measurement matter. I coach you on protein and resistance exercise, and InBody measurements let us follow muscle and fat changes over time.

Is hair loss a side effect of GLP-1 weight loss?

Temporary hair shedding, called telogen effluvium, can happen during significant weight loss. I can include hair support, such as Xtresse, and if you notice real thinning I can evaluate whether more support, such as nutrient optimization or hair restoration, makes sense.

What are the common side effects, and who should not take GLP-1 medications?

Common side effects include nausea, diarrhea or constipation, and fatigue. Most ease as your body adjusts, and smaller meals and good hydration help. GLP-1s are not suitable for everyone. People with a history of certain thyroid conditions, pancreatitis or specific allergies should avoid them, which is why a consultation comes first.

Are the medications compounded?

Compounded semaglutide and tirzepatide are used when they are clinically appropriate. They are made by a licensed 503A or 503B compounding pharmacy. Compounded medications are not FDA-approved and have not been evaluated by the FDA for safety, efficacy or quality. They are prescribed and monitored under the supervision of a licensed nurse practitioner and should be used only as directed.

Will I regain the weight after I finish?

Weight regain is possible if habits are not maintained. Maintenance may require ongoing medication, strength training, nutrition, monitoring and habit support, which is why I plan it with you from the start rather than at the end.
